Travel Permits



Travel Outside of Alberta (within Canada)

Submission of Travel Permit Request, Roster and Payment Required

  • Travel Permit - Outside of Alberta (within Canada) - Note: you must save the travel documents to your computer prior to filling in the fields
  • Please provide release letters for any player not registered with your team  - Handwritten rosters not accepted
  • Travel Permit Addendum: ONLY teams participating in any kind of training camp and/or exhibition games which are not a part of a sanctioned tournament are required to fill in page 3 of the Travel Permit
  • Cheque for $15 payable to Calgary Minor Soccer Association (CMSA)
  • Requests need to be submitted to CMSA a minimum of 2 weeks prior to departure
  • Late Travel permits will be subject to a $50 administration charges
  • No changes permitted to rosters one week before departure ($50 fee may apply)
  • Completed forms may be submitted in person, faxed to 403-236-3669 or scanned/e-mailed to Admin3@CalgaryMinorSoccer.com 
  •  Payment in person by cash, cheque, Visa, MasterCard, Debit card or over the phone during regular office hours by Visa or MasterCard
 

Travel to the United States

Submission of Travel Permit Request, Roster, Mandatory Travel Insurance Registration Form, and Payment Required

  • Travel Permit - To the United States Note: you must save the travel documents to your computer prior to filling in the fields
  • Please provide release letters for any player not registered with your team - Handwritten rosters not accepted
  • Travel Permit Addendum: ONLY teams participating in any kind of training camp and/or exhibition games which are not a part of a sanctioned tournament are required to fill in page 3 of the Travel Permit
  • USA destination(s) require a cheque for $120 plus Mandatory Insurance Fees payable to Calgary Minor Soccer Association (CMSA)
  • USA requests need to be submitted to CMSA a minimum of 4 weeks prior to departure
  • Late Travel permits will be subject to a $50 administration charges
  • No changes permitted to rosters one week before departure ($50 fee may apply)
  • Completed forms may be submitted in person, faxed to 403-236-3669 or scanned/e-mailed to Admin3@CalgaryMinorSoccer.com
  • Payment in person by cash, cheque, Visa, MasterCard, Debit card or over the phone during regular office hours by Visa or MasterCard 
 

International Travel

Submission of Travel Permit Request, Roster, Mandatory Travel Insurance Registration Form, and Payment Required

  • Travel Permit - International Travel Note: you must save the travel documents to your computer prior to filling in the fields
  • ASA Travel Insurance Form - International Travel
  • Please provide release letters for any player not registered with your team - Handwritten rosters not accepted.
  • All International destination(s) require a cheque for $230 plus Mandatory Insurance Fees payable to Calgary Minor Soccer Association (CMSA)
  • International destination(s) need to be submitted to CMSA a minimum 8 weeks prior to departure
  • Late Travel permits will be subject to a $50 administration charges
  • No changes permitted to rosters one week before departure ($50 fee may apply)
  • Completed forms may be submitted in person, faxed to 403-236-3669 or scanned/e-mailed to Admin3@CalgaryMinorSoccer.com
  • Payment in person by cash, cheque, Visa, MasterCard, Debit card or over the phone during regular office hours by Visa or MasterCard 

Guest Players

 CMSA Pins for Travelling Teams 

  • Teams can contact the CMSA Office if they would like to take pins with them to out of town tournaments. $10 for 20 pins.

If you require a copy of your CMSA roster for any sanctioned tournament within Alberta, please contact your soccer club.
